How Do You Know if Your Xbox Live Account is Being Hacked and How to You Avoid/ Fix It?

I think that there may be a slight possibility my XBOX Live account is being hacked/ stolen. I play mainly MW2 and have never, ever told anyone my passwords for the email accounts etc associated with my Live account. I went to play MW2 today, and for some reason, my clan tag was gone and sometimes, occasionly when I vote to skip a map, the vote sometimes doesn't go through. Like I said, I don't know for sure if it is being hacked, but how can you tell if it is, and is there any way to fix this or avoid it completely? By the way, I'm only like level 57 or 58 and haven't prestiged, so I don't think someone would want to hack my account (if this helps).

Suggestion:

My clan tag sometimes randomly disappears, same with the voting to skip a map, it's a small glitch but nothing to worry about. You'd be able to tell if your account was hacked because it wouldn't let you sign in to your account. If this ever happens, you need to contact Microsoft with your details (i.e e-mail address, username, password etc.)
